To help you consider how your life change will affect you and your Westlake benefits coverage, review the Information on Life Events chart under "Other Details". Any changes to coverage must be made within 30 days of the qualifying life event. Required documentation must be uploaded to your benefit enrollment portal within the 30-day deadline.

Dependent Eligibility and Verification

Dependent Eligibility and Verification

If you’re enrolled in any of Westlake’s benefit plans, you may also enroll your eligible dependents in that plan. For Westlake health, life and AD&D plans.


Your Eligible Dependents Include

  • Your legal spouse (as lawfully married under applicable state law) from whom you are neither divorced nor legally separated
  • Your children up to age 26, including your natural children, stepchildren, children for whom you have legal guardianship (including foster children) and adopted children.
  • Your children age 26 or older who are mentally or physically disabled and who rely on you for support and care.

It is important to verify that all of your enrolled dependents meet each plan’s required eligibility criteria.

When enrolling eligible dependents, you will need to upload the following required documentation to your benefit enrollment portal within 30 days of date of hire:

  • Certified marriage certificate

For covered children:

  • Certified birth certificate
  • Certified adoption paperwork
  • Certified legal guardianship paperwork
